Comprehensive discharge planning for your return to home and community begins upon arrival at Laurel Woods. Through our home management program, we help ease the transition from rehabilitation to independence. Our spacious therapy suite is equipped with a working kitchen and bathroom where you can build and practice your daily living skills before going home. Our therapy and social service professionals will evaluate your home environment to access the special challenges you'll face there. We'll introduce you to community services to further assure your continued well-being. We also offer instruction and guidance to your family members so they can better assist you upon your return home.
